Tooth sensitivity: why it zings, and what actually helps

That sharp, electric jolt from cold water or a sweet has a precise mechanism, and understanding it tells you exactly which remedies can work and which cannot. It also tells you how to distinguish harmless sensitivity from a nerve that is dying — a distinction worth several thousand rupees.
The mechanism, in one paragraph
Under your enamel lies dentine, which is not solid: it is threaded with millions of microscopic tubules running from the outer surface to the nerve. When dentine is exposed, changes in temperature, pressure or osmotic concentration make fluid move within those tubules, and that movement triggers the nerve. Hence the sharpness, and hence the fact that it stops the instant the stimulus goes. Everything that treats sensitivity either plugs the tubules or calms the nerve.
Why dentine gets exposed
- Gum recession — the root has no enamel at all, only a thin layer of cementum that wears off quickly. This is the most common cause in adults. See gum disease.
- Over-brushing — a hard brush plus scrubbing plus force notches a groove at the neck of the tooth. Almost always worse on the side opposite your dominant hand.
- Acid erosion — citrus, aerated drinks, vinegar-based foods, frequent lime water, and reflux. Acid softens enamel; brushing straight afterwards then scrubs it away.
- Grinding and clenching — wears through enamel and flexes the tooth at the neck.
- Recent treatment — scaling, a new filling or whitening all cause temporary sensitivity.
- Decay or a cracked tooth — these are not simple sensitivity, and they will not respond to desensitising toothpaste.

Sensitivity has several different causes — worn enamel, receding gums, grinding, a crack or hidden decay — and what genuinely helps is different for each. What a dentist can add
In-clinic fluoride varnish, sealants or bonding agents applied to the exposed neck, a composite filling to cover a deep notch, a night guard for grinding, and gum grafting in selected recession cases. Where sensitivity is caused by decay or a crack, treating that is the answer.

How long does sensitive toothpaste take?
Two to four weeks of consistent twice-daily use. Rubbing a smear onto the spot at night and not rinsing speeds it up noticeably.
Is sensitivity after scaling normal?
Yes. Removing calculus exposes root surfaces that were covered, and they take a few weeks to settle. Use a desensitising toothpaste through that period. Sensitivity that worsens rather than improves needs review.
Why only in winter or with the fan on?
Cold air produces the same fluid movement in the tubules as cold water. Sensitivity to air is a classic sign of exposed dentine at the necks of the teeth, usually from recession or abrasion.
Can it go away permanently?
Often yes, if the cause is addressed — technique corrected, acid intake reduced, gum disease treated, grinding managed. If dentine stays exposed it can recur, and ongoing use of a desensitising toothpaste keeps it controlled.
